Preflight Your Appointment

To help your appointment go smoothly, please bring:

  • Government-issued photo ID

  • Your MedXPress confirmation number

  • Printed copy of your 8500-8

  • Glasses or contact lenses, if worn

  • A current medication list

  • Any relevant specialist notes, test results, or prior FAA correspondence

  • CPAP compliance report, operative reports, cardiac testing, or other records if applicable to your medical history

Conditions That May Require Additional Review

Some medical conditions do not automatically prevent certification, but they may require more documentation or FAA review. These can include:

  • Sleep apnea

  • ADHD history

  • Anxiety, depression, or SSRI use

  • Cardiac history
